Referral Process
CREST Collaborative accepts referrals from both member and non-member districts on a rolling basis. We typically have openings year-round for students with valid IEPs, those requiring Extended Evaluations, and occasionally Interim Alternative Educational Placements (IAEP).

The Student Referral Process:
The referral process begins upon receipt of a complete referral packet from the sending district.
Required Documentation
The referral packet must include the following:
504 or IEP: active/signed and/or proposed IEP
Evaluations: Most recent reports
Academic Records (if applicable): Most recent progress report, report card, and/or transcript
Assessment Scores: MCAS score report
Supplemental Behavioral Documentation (if applicable)
If the student engages in self-injury, aggression, property destruction, and/or elopement, the packet must also include:
Conduct log and a Behavior Support Plan
If the student requires de-escalation and/or restraint, the packet must also include:
Incident/Restraint reports
If the student has been suspended, the packet must also include:
Suspension documentation and Manifestation documentation
